Our Specialist Support Coordination Services include:

• Plan Implementation and Management:

  Our Specialist Support Coordinators assist participants in understanding and implementing their NDIS plans. They work collaboratively with participants, their families, and support networks to identify and connect with suitable service providers, ensuring that participants receive the supports outlined in their plans.

• Coordination of Supports:

  Our Specialist Support Coordinators coordinate various supports and services to ensure a holistic approach to participants' care. They liaise with support providers, therapists, healthcare professionals, and other relevant stakeholders to ensure effective communication, seamless service delivery, and alignment with participants' goals.

• NDIS Plan Review and Renewal:

  Our Specialist Support Coordinators guide participants through the NDIS plan review and renewal process. They assist in collecting relevant information, documenting progress, and preparing for plan reviews to ensure that participants' changing needs are accurately reflected in their new plans.

• Capacity Building:

  Our Specialist Support Coordinators focus on building participants' capacity and independence. They provide guidance, coaching, and training on self-advocacy, decision-making, and navigating the NDIS system. They empower participants to actively participate in the management of their supports and build skills to effectively utilize their NDIS funding.

• Advocacy and Appeals:

Accessing Specialist Support Coordination Services under the NDIS:

To access our Specialist Support Coordination services under the NDIS, individuals must have an approved NDIS plan that includes funding for specialist support coordination. Participants may be allocated this funding if they have complex needs or require additional assistance in managing their supports.
